Service Dog Application

Before you begin

This application takes about 15 to 20 minutes and cannot be saved partway through, so please have the following ready before you start:

• Your contact and address details, and your date of birth.
• Service history: branch, dates of service, MOS or role, and VA disability rating (veterans), or department, role, and years of service (first responders).
• The conditions and specific tasks you hope a service dog would help with.
• Household details: housing type, whether you own or rent, who lives with you, other pets, and your typical daily activity level.
• DD-214 or proof of service (digital file ready to upload).
• A high-resolution photo of yourself for media use.
• A provider's letter, if you have one (optional).
• A landlord permission letter, if you rent.

You will also be asked to read and electronically sign a Media Release and a Release of Liability, and to acknowledge that, if approved, a $350 application fee is required to proceed. Accepted file types and sizes are noted on each upload field.
